Insurance codes are set by the Canadian Dental Association (CDA) and are contained in the Uniform System of Coding and List of Services (USC&LS). The Ontario Dental Association (ODA) conforms to the USC&LS selectively.

Practitioners in Ontario may choose to invoice the patient directly, or to accept assignment from the insurer.
